Healthlink Care Coordinator - Greater Chattanooga Area
The Healthlink Care Coordinator for Hamilton County in the Greater Chattanooga Area serves as the primary care coordinator that provides case coordination using an integrative approach to a pre-determined number of children and/or adults. They function as a member of the treatment team to plan, implement, and evaluate successful interventions for children, adults, and/or families. They will interface with community agencies and referral sources to coordinate care, as well as to increase cooperation and consistency between home, school, community, and physical health providers. They will provide advocacy, linkage, and referral services as need, participate in direct supervision with the program supervisor and RN Coordinator as necessary, and participate in treatment team meetings with a RN Care Coordinator and/or psychiatrist/APN. They will also maintain appropriate chart records and complete documentation in a timely manner, provide medication monitoring, provide clients with therapeutic support and crisis intervention when needed, and uphold Center policy and procedures, as well as CARF standards. The holder of this position will provide contacts with individuals that meet the requirements of the program.
The Healthlink Care Coordinator must have the ability to effectively and ethically coordinate care for children, adults, and/or families. They must be able to present professionally and be able to work within a team format to plan, implement, and evaluate successful interventions. This person must be able to work within a team format to meet positive goals for the clients. They must have the mental ability to exercise sound judgment under pressure, the ability to demonstrate effective decision-making skills, the ability to set and then demonstrate appropriate boundaries, the ability to be an empathetic listener, the ability to be flexible, demonstrate willingness, and be adaptable when working with diverse populations. They must also be able to communicate effectively and possess strong time management and organizational skills. They must embrace the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.
Education/Knowledge: A bachelor's degree in a health-related field of counseling, psychology, or social work. Candidates with a bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting.
Experience: Course work and/or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental retardation is required. Computer experience is helpful for this position.
Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. Healthlink Care Coordinators must possess reliable transportation they can conduct Center business in and also requires that the employee has an F Endorsement on his/her Driver License to be available to transport individuals as needed.Maintaining a dependable vehicle and certified driver status is a condition of employement.
Physical: The ability to hear normal and soft tones as well as complete close eye work. The ability to lift up to 50 lbs. There is frequent sitting, standing, walking, bending, stooping, and reaching in this position. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking. Physical de-escalation techniques will only be implemented at Helen Ross McNabb Center facilities, and not in community settings.
Salary range for this position starts at approximately $15.02 per hour depending on relevant experience and education.
