The PSH program serves the most vulnerable households in the homeless community through intensive case management and the Housing First principle.

Job Description

As a non-profit social service agency serving those in need, the Chattanooga Community Kitchen (CCK) offers essential, preventive, and permanent-housing programs to address the causes, experience, and risk of homelessness.

Reporting to the Director of Case Management, the Permanent Supportive Housing (PSH) Case Manager of CCK provides case management services to people engaged in the program, assists participants in securing permanent housing, and provides referrals for supportive services to community partners, all with the intent of maximizing self-sufficiency and moving people into housing as quickly as possible.

As an Outreach Specialist, the PSH Case Manager:

• Assesses potential participants in the Continuum of Care for vulnerability and program eligibility through triage assessments, vulnerability interviews, and coordinated entry

• Performs outreach to encampments, shelters, the CCK Day Center, and other service providers within the Continuum of Care (CoC) service area

• Develops and maintains a referral network of regional social service agencies to identify potential program participants

• Schedules program entry appointments for intake interviews

As a Case Manager, the PSH Case Manager:

• Assesses, performs intakes, and works with people to address barriers to housing, creating individualized case plans to achieve goals; provides follow-up case management

• Makes referrals for necessary resources and housing options as appropriate

• Plans and conducts regular life-skills classes

• Maintains current case files on each program participant

• Accurately and timely enters participant data into the Service Point HMIS database

• Creates and submits monthly reports including summary reports for persons served

• Implements evidence-based best practices throughout the program

As a Team Member and Agency Representative, the PSH Case Manager:

• Works cooperatively with partner agencies to provide exceptional service to participants

• Represents CCK at community functions, networking events, and other dealings to promote the work of CCK while educating those needing services about programs

• Ensures that all participants served by the PSH program receive excellent service delivery and are treated with dignity and respect

• Attends and contributes to supervisory, staff, and advisory meetings as requested

• Performs other duties as assigned


• Highly intelligent with 4 year college degree from an accredited institution, preference for social service related degree and higher education

• 1 year case management experience; housing case management preferred

• Preference for LBSW or higher licensure

• Understanding of HUD grant reporting is a plus

• Occasional flexibility in scheduling

• Computer literacy to include Microsoft Office (Word and Excel)

• Attention to detail in reporting and completion of case notes

• Ability to handle crisis situations while maintaining professional composure and confidence

• Compassion for and desire to work with those facing homelessness

• Shared belief in the mission and work of the Chattanooga Community Kitchen
