Job Title: Licensed Clinical Social Worker (LCSW)
Reports to: Nurse Practitioner/Director of Clinic Services
FLSA Status: Salary, Exempt
The Licensed Clinical Social Worker (LCSW) reports to the NP/Director of Clinic Services. The Licensed Clinical Social Worker or LCSW is responsible to improve the quality of life for persons living with HIV/AIDS, HCV or STI’s. The incumbent is responsible for the overall coordination of psychosocial services for an assigned caseload. Regular assessments are conducted through home or office visits as well as ongoing support in maintaining linkage to federal, state, local and community providers. The LCSW works effectively with the physician(s), clinic team and support services team to assure mission, goals and performance standards for the clinic and support services are met. The LCSW should understand his/her own limitations, seeking advice and assistance when necessary. Responsibilities/Accountabilities (includes, but not limited to):
• Works closely with all members of the healthcare team in development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
• Provides positive customer relations in dealing with clients/patients, their families, physicians, members of the healthcare team, referral resources, services agencies, all Cempa staff, and others.
• Develops and maintains thorough knowledge of referral resources throughout the service area.
• Conduct client intake interviews and related intake assessments, including understanding of the clinical diagnoses, collection of psychosocial information, and completion of intake forms and documentation.
• Assist in all aspects of Social Work Case Management for clients, including, but not limited to referrals; supporting linkage maintenance and consistency; coordination and reduction of service redundancies; client advocacy; and development of psychosocial care plan.
• Complete 60-day assessments to monitor clients’ community stability and facilitate necessary referrals.
• Provide 600 comprehensive ‘individual’ outpatient mental health sessions per year.
• Collect client data and information using specifically designed formats, in accordance with Ryan White Care Act funding requirements.
• Manage and assist in the resolution of client crisis.
• Complete documentation thoroughly and in a timely manner, in the medical record and eClinical.
• Participate in internal quality assurance reviews.
• Participate in team conferences.
• Maintain sensitivity and interest to the unique diversity found in the Chattanooga area, including, but not limited to people of color, diverse cultural and religious backgrounds, economic levels, and/or LGBTQQI community members.

Essential Functions and Skills (includes, but not limited to):

• Must demonstrate excellent communication skills.
• Must be able to collaborate well with others, since they interact with professionals from many different social service and government agencies.
• Must have emotional resilience and stability since the work often involves dealing with crisis.
• Must have knowledge in caring for patients in different developmental stages throughout the life cycle.
• Consistent attendance is required to assure that Cempa client’s needs are met.
• Must be able to handle and maintain confidential and proprietary information using discretion in all matters, and high level of integrity.
• Demonstrated ability to successfully work in a team environment with collaborative skills.
• Master’s degree in Social Work (MSW) or Counseling required.
• License as an LPC/MHSP or LCSW in the State of Tennessee.
• CPR Certification.
• 5+ years’ experience in outpatient mental health experience.
• Experience with Electronic Medical Records system; eClinicalWorks, CAREWare, specific but not required.
• Experience in working in a HIV/AIDS, Hep C, STI’s environments.
• Accrue at least ten (10) contact hours of continuing education each year.
